BFS Cheq - Cheque Cashing & Bank Cheque Issue Authority Form (24KB)
BFS Cheq account clients should use this form to establish, amend or cancel an Authority with Westpac Bank to be able to cash a cheque or to exchange a cheque for a Westpac Bank Cheque.

Use this form to advise a change of address / name / interest / account name

Please use this form to accompany any funds forwarded to a BFS State Office for investment. Please make all cheques payable to "Baptist Financial Services Australia Ltd"
Stop Payment & Indemnity - Corporate Cheque, Bank Cheque or International Cheque Only (22KB)
Please use this form to arrange with BFS to Stop Payment with its banker/s in respect of a Corporate Cheque, Bank Cheque or International Cheque which has been lost or stolen.
Stop Payment Notice - BFS Cheq Account only (20KB)
Please use this form to request a Stop Payment in respect of a cheque drawn on a BFS Cheq account.
Term Investment Renewal Notice (26KB)
This form is sent to clients at least 7 working days prior to maturity of a term investment for instructions to renew the investment.
Westpac Business Express Deposits for BFS Cheq Business A/cs (117KB)
Business Express Deposit satchels from Westpac Bank are available without charge for business clients enabling cash to be lodged at a Westpac branch through their quick deposit facility
Merchant Card Services Application (24KB)
Clients who want to accept credit card payments for deposit to their account should use this form to send to the Merchant Services Provider who then will contact you directly with details of options available and costs.
Please contact BFS before sending this form to the Merchant Services Provider, or send BFS a copy of the form.
Withdrawal Form (25KB)
Use this form to withdraw or transfer funds from an at Call or BFS Cheq account, and forward to a BFS State Office.
